Title: Clinicians’ adherence to the updated CPG for BPPV

Abstract

Background: Benign paroxysmal positional vertigo (BPPV) is the most common cause of vestibular vertigo. Variations in clinical practice regarding its management among clinicians have been noted in previous studies. The purpose of this study was to evaluate the Saudi clinicians' adherence to the updated clinical practice guideline (CPG) for BPPV and to investigate the variations in the adherence between different specialties.

Method: An online-based survey was developed and used to measure the clinicians' adherence. Participants were asked to make their management choices based on hypothetical patient vignettes.

Result: A total of 77 clinicians from different specialties were enrolled in the study. The respondents’ mean adherence score according to the vignettes was 63% (sd = 11.57).

The result of an independent-samples t-test indicated that adherence scores for Otolaryngologists, M(sd) = 68.57 (9.57) were significantly higher than for clinicians from different specialties, M(sd) = 60.90 (11.68), p = .006. It also indicated that clinicians who have a postgraduate degree, M(sd) = 66.14 (11.61) were higher in adherence than those with a bachelor's degree only, M(sd) = 59.68 (10.6), p = .014. In addition, those who were aware of the existence of the CPG had higher adherence scores, M(sd) = 66.62 (10.17) than those who were not M(sd) = 55.47 (11.04), p < .001.

Conclusion: Otolaryngologists were more adhered to the CPG compared to other specialties. Among all specialties higher adherence were associated with familiarity with the guideline, and a postgraduate degree.
