Challenges of sphenoid - frontal endoscopic surgery at Hong Ngoc hospital
Abstract
The sphenoid – frontal sinus and related anatomy should be known considering the intimate relationships with vital surrounding skull base structures, such as the internal carotid artery, the optic canal, the obit, the olflactory, maxillary and vidian nerves etc.
The sphenoid – frontal sinus has numerous variations regarding the pneumatization, the septation and the relation with the neurovascular structures. Multiplanar sinus CT scan and MRI are mandatory in the preoperative evaluation of the patients to avoid injury during endoscopic surgery.
In this article, we review and describe the importance of the anatomical landmarks of the sphenoid and frontal sinus on the CT scan, the anatomic findings that augment the risk of complications during sphenoid – frontal sinus endoscopic surgery, such as cerebrospinal fluid leak, visual loss, extraocular muscles palsy, intracranial haemorrage, loss of smell etc.
There were no major complications. Postoperative complications included frontal recess stenosis, polypoid mucosa recurrent etc. Moreover, we discuss about the appropriate type of endoscopic approach of the sinus depending on the localization of the pathologic process as well as the endoscopic surgery challenges.
We analysed the patients about fungus, mucocele, osteoma, papilloma, angiofibroma (JNA) etc who addressed to our ENT department in the last 6 months and who underwent CT and MRI of the paranasal sinuses and we studied the pneumatization type and the anatomic variations of the sphenoid – frontal sinus. A variety of sphenoid – frontal sinus anatomy variations were found, and we emphasize the value of imagining evaluation before performing functional endoscopic sinus surgery.
Biography
I have completed my Otorhinolaryngology master degree in 2006 at Hanoi Medical University, Vietnam. After that, I worked and held the position of head ENT department at Hong Ngoc general hospital until now. During this time, I have been working and learning continuously such as otology, sinus and skull base surgery in Malaysia, Singapore, Korea, United Kingdom etc. And completed my graduation of Otology specialist degree II in 2022 at Thai Nguyen Medical University, Vietnam as well.
I am so honored to be a member of the charity surgical society “Facing The World” is headquartered in London, UK and has participated in charity surgery programs for unlucky patients in Vietnam as well as Cambodia.
I am one of a speaker at the 20th Asian ORL-HNS congress with the topic of Endoscopic Ear Surgery held in Hanoi, Vietnam, 2023 and 2nd International conference on Otolaryngology research and treatment at Bangkok, Thailand, 2024.
